The Template for Debriefing Following a Student Error Using Reflection and Quality and Safety Competencies integrates processes of Just Culture for application with student errors. Items 1-10 are actions to address individual accountability with a student’s error. The second page items 1-4 are actions to address system accountability within the nursing program.
